A Home Care Package (HCP) is a government-subsidised program designed to help you live independently at home. It provides funding for services that support your care needs, ranging from personal care to home modifications and allied health services, like physiotherapy.

 

2. What Happens Next?

Now that you’ve been approved for a Home Care Package, here’s a step-by-step guide to getting started:

Choosing a Provider:

The first step is selecting a Home Care Package provider who will manage your funding and work with you to coordinate your care services. There are many providers to choose from, and it’s important to find one that understands your needs and preferences.

Understanding Your Level of Care:

Your Home Care Package is tailored to your individual needs, with four levels of funding based on the amount of support required:

  • Level 1 (Basic care needs): Approx. $10,271 per year.
  • Level 2 (Low-level care needs): Approx. $18,063 per year.
  • Level 3 (Intermediate care needs): Approx. $39,310 per year.
  • Level 4 (High-level care needs): Approx. $59,593 per year.

Each level has a different amount of funding, and this funding will determine what types of services you can access. The higher the level, the more funding is available to support your care needs.

 

3. What Can My Package Be Used For?

Your HCP can cover a wide range of services, tailored to your needs:

  • Personal Care: Assistance with daily tasks such as showering, dressing, and mobility.
  • Allied Health: Including physiotherapy, occupational therapy, and podiatry.
  • Home Modifications: Changes to your home to make it safer, such as installing rails or ramps.
  • Social Support: Help with accessing the community, social activities, and transport.

Your provider will help you create a care plan that maps out which services you will receive.

 

4. How Do I Choose the Right Provider?

Research Providers:

Look at what different providers offer, including their fees, services, and flexibility. Some providers offer a wider range of services or specialise in certain areas, so choose one that suits your specific needs.

Ask Questions:

Make sure to ask potential providers about how they’ll support your specific goals and preferences. Some questions might include:

  • What services are included in my care plan?
  • Can I change my services later if my needs change?
  • What are the fees and charges associated with managing my package?

 

5. How to Develop Your Care Plan:

  • Once you’ve chosen a provider, you’ll work with them to develop a care plan. This plan will outline the services you’ll receive, how often, and who will provide them.
  • Personalising Your Care: Ensure the care plan reflects your individual needs and goals. If, for example, you’ve been receiving private physiotherapy, you can discuss continuing this as part of your funded services.

 

6. Your Rights Under the Consumer Directed Care (CDC) Model:

Understanding your rights under the Consumer Directed Care (CDC) model is essential for taking full advantage of your Home Care Package (HCP) and ensuring you receive the care that best meets your needs.

What is Consumer Directed Care?
  • Consumer Directed Care (CDC) is an approach to aged care that empowers you, the consumer, to have greater control over your care services. It emphasises your preferences, goals, and choices, allowing you to tailor the care you receive to suit your individual needs.
  • CDC promotes a partnership between you and your care providers, ensuring that you are actively involved in decisions regarding your care and support. This approach recognises that everyone’s needs are unique, and flexibility is key to achieving positive outcomes.
Flexibility and Control:
  • The CDC model means you have the power to make choices about your care. This includes deciding who provides your services and how your funds are spent.
Freedom to Choose Your Providers:
  • If you already have health professionals or service providers you trust (e.g., your current physiotherapist), you have the right to ask your HCP provider to set up a service agreement with them to continue your care.
Changing Providers:
  • If, at any time, you feel the provider is not meeting your needs, you have the right to switch providers or change services.
What This Means for HCP Providers:

HCP providers must inform clients of their rights to choose service providers under the CDC model. Failure to do so could potentially breach their obligations under the Aged Care Act and Aged Care Quality Standards, which could result in regulatory action from the Aged Care Quality and Safety Commission if a complaint is made.
